Our doctors are always prepared. Because of this, we require all new patient paperwork to be filled out completely at least one week prior to your appointment. This allows us ample time to review your medical/dental history and to look into any dental insurance benefits you may have.
Please set aside at least 30 minutes to complete these intake forms thoroughly and honestly. Doing so can help avoid medical/dental complications, unexpected out-of-pocket expenses, and delayed treatment.
Appointments with intake forms not completed within one week of your appointment date will unfortunately be canceled and rescheduled.
If you have x-rays/records from your previous dentist, please contact them directly and request that they be sent to info@450aesthetics.com.
